Dying Well Community Charter

Birmingham CrossCity CCG has been awarded Pathfinder Community status by the National Council for Palliative Care.

The Dying Well Community Charter is a commitment by individuals, communities and organisations to work together to ensure that people are well supported and cared for through dying, death, grief and bereavement.

Birmingham CrossCity, together with its neighbour, Birmingham South Central CCG, is one of eight Pathfinder Communities to have been awarded the status, and will promote the charter in a number of ways, including raising awareness of the importance of good end of life care; developing local partnerships to improve the service; and sharing experience to help other parts of the country.

Last year Birmingham CrossCity and Birmingham South Central CCGs held a public consultation about their plans to improve palliative and end of life care so that patients get the right care, in the right place, at the right time, and more people reaching the end of their lives will be able to choose where they spend their final days.
