This enhanced standard increases the number of services available within primary care and promotes a localised service, meaning patients can access more services from their GP instead of having to go to hospital. It sets out a package of care which can be delivered by individual practices, or groups of practices working together should they not have the expertise and capacity required to reach this level.
Our member GP practices can sign up to offer these services which will support patients to receive care closer to home and improve continuity. This means services will be offered within the patient's practice or a neighbouring practice, potentially meaning that patients will not have to travel to hospital to have certain tests or investigations.
The ACE Excellence level will include:
- Easy access to high quality diagnostic investigations such as ECG, spirometry and insulin initiation within primary care
- Improved primary care access to wound management services for patients discharged from hospital or specialist care providers following surgery or trauma, including discharges from A&E
- Improved patient and carer experience by providing care closer to home
- Preventing unnecessary referrals to secondary care by offering more support for patients with long term conditions within general practice
